A case summary form with all the relevant information about the case we may need. This will help you and us organise the information more easily.
The DICOM images of the relevant imaging studies that have been performed for this case. We will share with you a link by email to upload this material.
A list of specific questions you want to answer about the case. Examples are included in the case summary form for you.
